DCIS is NOT Cancer!

“DCIS is not cancer, so why are we calling it cancer and always treating it like it is cancer?”

These are the words of Dr. Laura Esserman, Director of the Carol Franc Buck Breast Care Center at the University of California, San Francisco. She posed this question while speaking at the Australian Breast Cancer Conference in Melbourne.

Boxout_Laura_Esserman_400_413-(1)“DCIS is not an emergency,” Dr. Esserman said. “It’s something that creates an environment where cancer may arise over the next 10-20 years, yet patients are being told they need surgery in the next fortnight.”

Dr. Esserman is a member of an expert US panel advising the National Cancer Institute which, in July this year, published recommendations in the Journal of the American Medical Association that included a call to rename precancerous conditions such as DCIS.

They recommended that new terminology should be used that does not include the word “cancer” so that patients would be less fearful and less likely to have potentially unneeded and harmful treatments, including surgery and radiation.

Dr. Esserman said there was clear evidence that a watch-and-wait approach, especially if combined with medicines that prevent hormone positive breast cancer, is appropriate for low-grade DCIS.

My 2 Hour Consultation with Dr. Gordon Saxe, Center for Integrative Medicine, UCSD

Dr. Gordon Saxe is the Director of Research, a preventive and integrative medicine physician, a founding member of the UCSD Center for Integrative Medicine, and co-developer of the UCSD Natural Healing & Cooking Program. He is also the recipient of a prestigious NIH Career Development Award from the National Center for Complementary and Alternative Medicine. Dr. Saxe is a national expert in cancer and complementary and alternative medicine, and most well-known for his pioneering work in the combined use of a plant-based diet and body-mind stress reduction to control the progression of advanced prostate cancer.  His previous studies have included: epidemiology of diet and cancers of prostate, breast, and pancreas; diet and body-mind exercise to control spread/of advanced prostate cancer; and diet and gene expression in prostate cancer. His background in complementary and alternative medicine includes study of: nutritional healing: diet, vitamins/minerals, herbals, and other non-drug medicines; exercise modalities: yoga, t’ai chi, and other body-mind approaches; and eastern and western systems: Chinese medicine, Ayurveda, homeopathy, macrobiotics, and natural hygiene.

Intrigued, I continued to investigate and stumbled on some youtube videos.

I decided to make an appointment…

“I’m a big fan of yours,” I said when Dr. Saxe came in to greet me. “I’ve watched a few of your videos and I think the work you are doing is great!”

For the next two hours, Dr. Saxe took notes on a yellow pad as I explained to him my DCIS Journey – beginning with a suspicious mammogram in August 2009.

I began, “When I was diagnosed with DCIS in January, 2010, I was shocked by what I was told were my options: (1.) Mastectomy or (2.) Partial Mastectomy plus 7 weeks of daily radiation.

I was handed a pamphlet called “A Women’s Guide to Breast Cancer Diagnosis and Treatment” and a file with 5 business cards stapled to it for making appointments over the next two weeks including: a breast surgeon, a radiation oncologist, a medical oncologist, an MRI and a cancer care coordinator. photo (31)

Then I was asked, ‘would you like a prescription for Xanax?’  

What would have been much more helpful,” I told Dr. Saxe, “was a referral for an integrative Dr. such as you. I think all patients given a DCIS diagnosis should be given a referral to see an integrative doctor.”

I told him integrative counseling should at least be offered – especially when someone like me is asking, “Is there anything I can do as far as changing my diet?”

Even though I am a Certified Nutritionist, I learned a few new things and got motivated to sign up for the Natural Healing and Cooking Class Dr. Saxe founded. Here are some more excellent recommendation Dr. Saxe gave me:

(1) Whole food, plant-based diet based on:
* Whole grains
* Vegetables
* Beans & legumes
* Fruit, seeds, nuts
* Small amounts of healthy, organic food of animal origin

(2) Foods for breast health and cancer prevention: Fermented soy foods (especially South River or Ohsawa miso), seaweed, brassica (cabbage) vegetables, mushrooms, green and kukicha tea

(3) UCSD Natural Healing & Cooking Program for education and support in making dietary changes.

(4) RPM – rise, pee, meditate!

I have invited Dr. Saxe to write an article for this blog.. . . perhaps an article on food-based “natural hormonal therapy.”

Active Surveillance Anyone?

Looking for women who are doing “active surveillance” of possible DCIS based on suspicious calcifications from a mammogram.

My friend and co-founder of www.dcisredefined.org Sandie Walters and I have recently been contacted by someone from an online “cancer” magazine who is looking to interview women who are doing active surveillance of suspicious breast calcifications. Please contact me at dp4peace@yahoo.com if you would be interested in being interviewed.

I consider myself to be in the “active surveillance club” since I had a group of calcifications I chose not to biopsy (despite recommendations in 2011.) I have been monitoring with RODEO MRI for the last 3 years and so far, so good!

Where are the studies being done for active surveillance? How about we start an “active surveillance club?”

My Latest MRI Results…

How many Medical Directors are gracious enough to give out their direct cell # and actually answer the phone to discuss MRI results with patients?

Praise for Aurora RODEO MRI, Dr. Reitherman and Memorial Care Breast Center in Laguna Hills, CA! It was certainly worth the 1 hour drive each way!!

Dr. Reitherman asked about my history. After sharing my story (low grade DCIS, 3 surgeries, a close margin, no radiation and no further mammograms), his response was, “While legally I still need to recommend mammograms as standard of care, I think an annual MRI is a reasonable approach.”

My approach to breast health is so much more than an annual MRI! It’s been four years and I am still happily following Donna’s Daily Rx.

Great Gratitude & Great “Finds”

thank youAs 2013 comes to a close, I want to express my heart-felt gratitude for all of the wonderful women who have contributed their wisdom, support, resources and personal stories on this site.

Four years ago, I felt like I was completely alone in my relentless questioning of the “standard of care” regarding DCIS.  Yet I knew there had to be many more women like me who were feeling the same uncertainty and wishing for better, healthier and saner solutions. I knew other women must be searching like I was.

I had no idea how to create a blog or website, yet in December of 2011, two years into my DCIS journey, I was “highly motivated.”  I felt compelled to share my personal experiences, thoughts and resources. DCIS 411 was created in less than a month. My goal was to be the website I had so desperately searched for, yet did not find.

It’s been four years for me now on this wild DCIS roller coaster ride and while I am still searching for better, saner solutions, I am so grateful for the many enlightening discoveries along the way.  One of my best “finds” was Sandie Walters who created a simple website called “DCIS Without Rads.” Over the last two years, Sandie and I have shared many emails, phone calls and resources. Together we created DCIS Redefined: Dilemmas, Choices and Integrative Solutions. May this website be one of your great “finds” along your DCIS journey.
